Full-Time Position

Case Manager (FT)

Our case managers are responsible for coordinating and supervising the care that our Direct Care staff provides our clients.  If you can meet the requirements below you can fill out our online application.

  • Requires a Bachelor's Degree (B.A., B.S.) in a human services field
  • One year of direct care work experience
  • Employees are required to pass a background study, provide evidence of a degree in related area, and have an operating vehicle, current driver's license and auto insurance (due to transporting clients)
  • Will be responsible for conducting site reviews as directed by Service Director
  • Will be responsible for performing all of the responsibilities of the Direct Care Professional position as necessary (including direct client supervision)
  • Will be responsible for directly supervising Direct Care Professionals including, but not limited to: directing work, conducting performance reviews, rewarding and disciplining employees, addressing complaints, and resolving conflicts
  • Will be responsible for positive interactions with clients, guardians, client families, representatives of other agencies, and other persons with whom the clients interact
  • Will be responsible for admission, orientation, and a 45-day review of new clients
  • Will be responsible for the development and maintenance of the Individual Program Plan (IPP) including, but not limited to: goal/objective writing, observation of program implementation, ensuring proper documentation, and service recommendations to the IDT
  • Will be responsible for preparation of client transfer and discharge summaries
  • Will be responsible for compliance with Minnesota Rule 201 (Foster Care Licensing) and keeping client Foster Care Licenses current for each site
  • Will be responsible for properly acting upon, documenting, and distributing Incident Reports
  • Will be responsible for the oversight, coordination, and proper documentation of all client health, medical, and psychiatric care
  • Will be responsible for performing On-Call services when assigned
  • Other duties, as assigned
  • Supports and participates in common teamwork; cooperates and works together with supervisors and co-workers

Rule 79 Case Manager (male*) (FT)

The Rule 79 Case Manager helps coordinate many services for our school age clients.  The qualifications and duties are listed below.  If you are interested please fill out an online application.

  • Requires a Bachelor's Degree (B.A., B.S.) in a human services field
  • One year of experience in a mental heath field is preferred
  • Employees are required to pass a background study, have an operating vehicle, current driver's license, and auto insurance (due to transporting clients)
  • Coordination of services for children with Severe Emotional-Behavioral Disorders (SED) who qualify for Case Management services as stipulated by Rule 79 under the Minnesota Department of Human Services
  • Will coordinate and integrate the delivery of children's mental health services on behalf of the child and child's family
  • Will be responsible for accurate and timely response to the information requests from outside agencies or individuals as appropriate

  • Will be responsible for regularly reporting to the Rule 79 Case Management Supervisor as to the current status of each client
  • Will complete all mandated paperwork as outlined in Rule 79 Case Manager responsibilities
  • Will be responsible to positive interactions with clients, guardians, client families, representatives from other agencies, and other persons with whom clients interact
  • Will be responsible for admission, orientation, and assessment of new clients
  • Will be responsible for the development and maintenance of the Individual Treatment Plan (ITP) including, but not limited to: goal/objective writing, observation of program implementation, and ensuring proper documentation
  • Will be responsible for performing On-Call services when assigned
  • Due to the nature of the clients, a male employee is needed

Direct Care Professional (PT)

Access is always looking for creative and caring employees.  We have a lot of great clients who are in need of assistance from friendly people like you!  As a DCP you might help a client with basic living skills or you might just hang out and have fun.  There is a wide variety of clients and a wide variety of responsibilities.  This is an exciting position that can be very rewarding.  Please fill out our online application if you are interested.  Some more information about the position includes:

  • Schedules built around applicants' availability and client needs
  • Weekend differential at 24-hour client sites for Sat/Sun day shifts
  • Sleep overnight stipends
  • Employees scheduled weekends work every other weekend
