Case Manager

Requirements

Applicants must:

  • Be 18 years of age or older
  • Have one year of Direct Care experience
  • Have a Bachelor’s degree (B.A., B.S.) in a human services-related area or related field and be able to provide evidence of that degree
  • Pass a background study
  • Have a current driver's license, auto insurance, and an operating vehicle

Responsibilities

Applicants will be responsible for:

  • Performing all of the responsibilities of the Direct Care Professional position as necessary (including direct client supervision)
  • Directly supervising Direct Care Professionals including but not limited to:
    • Directing work
    • Conducting performance reviews, including 6-month performance reviews upon completion of orientation period
    • Rewarding and disciplining employees
    • Addressing complaints
    • Resolving conflicts
  • Conducting site reviews as directed by the Services Director
  • Positive interactions with clients, guardians, client families, representatives of other agencies, and other people with whom the clients interact
  • Admission, orientation and 45-day review of new clients
  • The development and maintenance of the Individual Program Plan (IPP) including but not limited to:
    • Goal/objective writing
    • Observation of program implementation
    • Ensuring proper documentation
    • Service recommendations to the IDT
  • Preparation of client transfer and discharge summaries
  • Compliance with Minnesota Rule 201 (Foster Care Licensing) and keeping client Foster Care Licenses current for each site
  • The effective implementation of behavioral plans programming, if applicable
  • Properly acing upon, documenting, and distributing Incident Reports
  • The development and maintenance of the Risk Management Plan for each client
  • The oversight, coordination, and proper documentation of all client health, medical, and psychiatric care
  • Balancing client financial accounts and maintaining accurate financial records for each client
  • Supporting and participating in common teamwork, including working together with supervisors and coworkers
  • Using tactful, appropriate, and professional communication
  • Performing On-Call services when assigned
  • Other duties, as assigned
